Idependent Contractor Agreement

PURPOSE
The company expects all employees to conduct themselves in a positive manner to promote the
best interests of our customers, company, fellow employees and the general public, and will
enforce these expectations fairly and with regard to all employees.
The purpose of this policy is to do document employee infractions regarding corrective action.
This will enable uniform and consistent application with reason and fairness to all employees.

POLICY

The Primary Corrective Action Policy may impose the following corrective action on an employee                                                                                                  for his/her failure to comply with company / or federal requirements.

RESPONSIBILITY
Company management is responsible for administering the policy. Management will establish
and maintain communication with employees to ensure compliance. Employees are subject to
policy for non- compliance as prescribed. Employee’s failure to comply with management’s
request per the policy may be further corrective action, including termination.

PROCEDURE
Corrective action, as determined by the company, including termination, may be imposed on an
employee for doing one or more of the following activities:

DRIVER DAILY LOG VIOLATIONS:
1ST Critical Violation                            Verbal Warning + $250.00 

2nd Critical Violation                           Written Warning +$500.00

3rd Critical Violation                            Termination

1st Form &Manner Violation              Verbal Warning+$300.00

2nd Form& Manner Violation             $600.00

(Hours of service or falsification violation is defined as critical)
(Violation within any 12 consecutive month period.)

The owner of the company always retain the right to review and modify any policies for any
accidents and incidents.
